What are the types of IOLs used in cataract surgery?

Monofocal IOLs are the most commonly used lenses in cataract surgery. They provide sharp vision at one fixed distance-either near, intermediate, or far. Most patients choose distance vision and use reading glasses for close-up tasks.

Best for:
✔️ Patients who don’t mind wearing glasses for near or intermediate tasks
✔️ Those looking for a cost-effective option
✔️ Individuals with an active lifestyle who prioritize clear distance vision

Monofocal Plus IOLs offer the benefits of standard monofocal lenses but with improved contrast sensitivity and a slightly broader range of focus. They enhance vision quality in different lighting conditions, reducing glare and halos.

Best for:
✔️ Those who want a better-quality monofocal lens
✔️ Patients who drive frequently, especially at night
✔️ Individuals who prefer clear distance vision with minimal visual disturbances

EDOF lenses provide an extended range of vision, reducing dependence on glasses for intermediate tasks such as computer work, reading, and daily activities. They offer a smooth transition between distances without the visual disturbances of multifocal lenses.

Best for:
✔️ Professionals who need intermediate vision (computer work, reading)
✔️ Individuals who prefer a more natural vision experience
✔️ Those who want minimal night vision disturbances compared to trifocal lenses

Trifocal IOLs provide excellent vision at near, intermediate, and far distances, reducing or eliminating the need for glasses. These lenses are ideal for patients who want complete visual freedom for reading, driving, and screen use.

Best for:
✔️ Those who want the highest level of independence from glasses
✔️ Active individuals who frequently switch between different activities
✔️ Patients are comfortable with slight night vision disturbances (halos and glare).

Refractive IOLs are designed to correct refractive errors such as astigmatism while also improving overall visual quality. These lenses offer sharper, more natural vision and are customized to each patient’s needs.

Best for:
✔️ Individuals with astigmatism who want precise vision correction
✔️ Patients seeking premium vision quality after cataract surgery
✔️ Those who want to reduce dependency on glasses across multiple distances

More About Cataracts

FAQs

Cataracts are diagnosed based on symptoms like hazy vision and eye strain while reading, watching TV, or using a computer. Reduced vision on a Snellen’s chart test also indicates cataracts.

Clinically, doctors detect cataracts using a direct ophthalmoscope, where the lens appears hazy. Dilation provides a clearer assessment of the cataract’s grade. Additionally, photographing the retina can reveal an unclear image due to lens haziness, helping both the doctor and patient understand its severity.

Cataracts typically develop around the same age as one of the parents who had them and cannot be prevented. However, maintaining a healthy diet, good nutrition, and regular exercise may help slow down the aging process. Once a cataract has formed, it cannot be reversed.

Once a cataract is removed, it cannot form again. The natural lens affected by the cataract is replaced with an intraocular lens (IOL) within the capsular bag. However, a condition called posterior capsular opacification (PCO) or after-cataract can occur. This happens when certain cells grow onto the posterior capsule—the thin membrane that holds the artificial lens—causing haziness in vision.

Cataract surgery is completely painless. If a patient is anxious, mild sedation can be given. The procedure is typically performed under topical anesthesia, using only eye drops. In some cases, a small 1cc injection may be administered if the patient is likely to move their eye or may have difficulty cooperating during surgery. However, these injections are painless as the patient remains partially sedated.

Like any surgery, cataract surgery has some risks, though it is generally safe and painless with sedation. Possible complications include posterior capsular tear, which may prevent the planned lens implantation (a backup lens is always available), corneal swelling (striate keratitis) treated with eye drops, infection that could lead to vision loss if untreated, and Toxic Anterior Segment Syndrome (TASS), a reaction to surgical chemicals. However, these risks are rare, and cataract surgery remains one of the safest procedures.

Theoretically, cataract surgery can be done on both eyes simultaneously. However, I, Dr. Neha Dholakia, strongly advise against it. It is always safer to schedule the surgeries at least a day apart. In the rare event of an unforeseen complication, such as an earthquake or infection, operating on both eyes at the same time could put the patient at risk of losing vision in both eyes. Unlike LASIK, which is a surface procedure commonly performed on both eyes together, cataract surgery is not done on both eyes simultaneously for safety reasons.

Yes, surgery is the only effective treatment. In the early stages, changing eyeglass prescriptions may help, but as the cataract progresses, surgery becomes necessary.

What are the stages of cataracts?

  1. Early Cataract – Initial minor changes in the lens.
  2. Yellow Cataract – Lens develops a yellowish tint.
  3. Advanced Cataract – Increased haziness and vision impairment.
  4. Brown Cataract – Lens hardens and turns brown.
  5. Black Cataract (Kalamotya) – Cataract becomes black and stone-like, severely affecting vision.

Early detection and treatment ensure better outcomes.
